What is Intermittent FMLA?

Intermittent FMLA time is the periodic absence from work taken to care for a covered family member or for your own serious health condition.

How Do I Request Intermittent Leave Time?

You must notify your manager and Unum 30 days prior to a scheduled leave if the need for the intermittent leave is foreseeable based on planned medical treatment, or as soon as possible if not foreseeable.

To request intermittent FMLA leave, contact Unum at 866-371-5723 between 7:00 a.m. and 7:00 p.m. CST, Monday through Friday. You will need to provide:

  1. Your full legal name, social security number or employee ID number, home address, phone number, date of birth and marital status
  2. WUSTL’s Policy #: 714055
  3. Occupation (or job title) Supervisor’s name
  4. The reason for and expected duration of your request.

How Do I Report Intermittent Leave Time?

If you have been approved for an intermittent leave, you are required to report any leave time used.

You must:

  • Contact your manager or supervisor as soon as you know you will be out of work due to your intermittent leave.
  • Contact Unum as soon as possible and within 7 calendar days each time you need to take intermittent leave. Otherwise, your leave under the FMLA and/or State leave law, where applicable, may be denied. Report your intermittent leave time off through Unum by following the instructions below.
  • Enter your intermittent leave time off in Workday by following the instructions below.

Reporting Intermittent Leave Time in Workday

  • In Workday, navigate to the Absence application
  • Click on the Request Absence button
  • Select the days intermittent leave will be taken
  • Click the Request Absence button at the bottom of the page
  • Select one of the following time off types from the drop down menu
    • Intermittent FMLA – Sick (Scheduled)
    • Intermittent FMLA – Sick (Unscheduled)
    • Intermittent FMLA – Unpaid
    • Intermittent FMLA – Vacation
  • Select edit quantity hours per day and enter the number of hours
  • Enter any comments and click submit

Reporting Intermittent Leave Time to Unum

Report Intermittent Absence Time on the Web

  1. Login to Unum (www.Unum.com) using your username and password
  2. Select the option to Report Intermittent Absence
  3. From the list of leaves, select the leave you are reporting time against
  4. Enter the Start Date and End Date of your absence
  5. If you have an unpaid meal break, enter the amount of time — EXAMPLE: You miss a full day and your work schedule is 8:00-4:30 with a 30 minute meal break, you would enter 30 minutes as unpaid meal break so this would not be deducted from your entitlement
  6. Enter the Total Time Off for that day — EXAMPLE: 2 hours off in the morning and 1 hour in the afternoon, absence for that day must be entered as a total of 3 hours off

Save entry and record the System Confirmation Number for future reference

Report Intermittent Absence Time using your Mobile Device

  1. Login to the Unum Mobile App and select the appropriate claim to report intermittent time
  2. From the list of leaves, select the leave you are reporting time against
  3. Select the option to Report Intermittent Absence
  4. Enter the Start Date and End Date of your absence
  5. EXAMPLE: You miss a full day and your work schedule is 8:00-4:30 with a 30 minute meal break, you would enter 30 minutes as unpaid meal break so this would not be deducted from your entitlement.
